ALL guests MUST fill out this form prior to pool use

We have read the pool rules and agree to comply with them. We agree that the breaking of pool rules by any family member may result in forfeiture of pool privileges for the entire family. We also agree to release the Friday Island Homeowner’s Association (FIHA) and all its members from any and all liability arising from any injury or damages that I or my family may incur as a result of using the pool facilities.
